  • Duly-signed personalized canceled cheque specifying the IFSC code and policy holder name printed on it. 
     
  • Self- Attested and signed bank passbook copy/bank statement copy (with the latest one month’s transaction details)
  • NEFT form (as attached)
  • ID proof / KYC documents
         A) Masked Aadhar card.
         B) Pancard.
 
Whenever you are providing an Aadhaar card then it should be a Masked Aadhaar number which implies replacing of first 8 digits of the Aadhaar number with some characters like “XXXX-XXXX” while only the last 4 digits of the Aadhaar Number are visible. You can also hide the 8 digits by placing plain paper above them.
 
Post receipt of the requested documents we shall process your request within 10 days.
